We cannot customize date-of-birth and gender fields in the backend. Therefore, they also don't appear in the checkout form.

Magento itself does not display Date of Birth and Gender at checkout, these fields are shown at the customer registration page. Thus out of the box our checkout also does not display these options and to add them additional customization is required.

No. Customers are suggested to enter an email address. If there is already an account at the store with such an email, the system will recognize it and provide the appropriate suggestions.

Yes. First, you need to install the Customer Attributes module if you’re using Magento Open Source. There are 2 ways of adding an attribute. You can see more details in the Extension Settings section of the User Manual.

No,Payment Method - defines the default payment method. Extension supports a few payment methods and payment gateways: B2B Company Credit, NET.30, Authorize.net, Bambora APAC, NMI, PayPal Payflow, Affirm, Klarna, SplitIt payment support, Amazon Pay module, and Braintree Payments by Gene module.

I'd suggest that you check and make sure that 'Newsletter Subscription Option' values are enabled in the extension configuration at admin. Please note that the checkbox becomes available only when you enter an email address in the field.

How do I make it so that this checkout will keep the "My billing and Shipping address are the same" Selected. I need customers to have the same address for both and cant adjust it.

The extension provides 'Billing and Shipping Addresses are the Same by Default' = Yes/No option that should work for your requirements (see the attached screenshot). I'd suggest that you check and make sure that you are using the latest Smart One Step Checkout on your store.
